What Is a Functional Abilities Form?
The WSIB Functional Abilities Form helps communicate what a worker can safely do while recovering from a workplace injury.
Rather than focusing on a diagnosis, the form identifies abilities, limitations and precautions related to activities such as:

Sitting, standing and walking

Lifting, carrying, pushing and pulling

Bending, twisting, kneeling and reaching

Using stairs or ladders
-
Repetitive movements
-
Work hours and gradual return-to-work schedules

Temporary workplace modifications
This information can help an employer identify suitable or modified duties that are safe, productive and consistent with the worker’s current abilities.

The assessment will be tailored to your injury, your current stage of recovery and the physical requirements of your job.
Who Can Request the Form?
A Functional Abilities Form must be requested by either:

The injured worker, or
-
The worker’s employer
Please bring the form provided by your employer or WSIB to your appointment. You should also bring a description of your regular job duties and any modified duties your employer is considering.
Information for Employers
Employers may use functional abilities information to develop appropriate modified-work options.
The completed form focuses on the worker’s functional abilities and restrictions. It does not provide the worker’s diagnosis or other confidential clinical information.
Employers may contact Body Maison, with the worker’s written consent, when clarification of functional restrictions is required.

Form completion depends on the findings of the clinical assessment. A specific return-to-work status or restriction cannot be guaranteed before the assessment is completed.

Does my employer complete the Functional Abilities Form?
Your employer may complete the workplace and job-information sections. The clinical functional-abilities section must be completed by an eligible regulated healthcare professional.
Will my diagnosis be shared with my employer?
The form focuses on functional abilities, limitations and precautions. Diagnostic and confidential clinical information should not be included in the information provided to the employer.
Can the assessment recommend light duties?
The healthcare professional can describe your functional abilities and restrictions. Your employer uses this information to identify appropriate modified or suitable work.
Is this covered by WSIB?
WSIB may pay its established fee for completion of an eligible WSIB Functional Abilities Form. Assessment, treatment or completion of an employer-designed form may follow different billing rules. Coverage should be confirmed before the appointment.
Pricing
A Functional Abilities appointment includes($200):
- Initial Assessment: Regular initial assessment fee $150
- WSIB Functional Abilities Form Completion: Additional $50
The form fee is added to the cost of the initial assessment and covers reviewing the assessment findings, documenting functional abilities and restrictions, and completing the applicable form.
Please bring the Functional Abilities Form, your job description and information about any proposed modified duties to your appointment.
What happens if my abilities change?
A follow-up assessment may be recommended to update your restrictions, gradually increase your duties or support your return to regular work.
Can you guarantee that I will be cleared for work?
No. Recommendations are based on the healthcare professional’s assessment findings and the functional demands of your job.
